Erie Family Health Center Navigates Care Across the Chicagoland Area

erie family health centerWith the recent changes to insurance and managed care due to the Affordable Care Act and the state of Illinois switching Medicaid from a "fee for service" system to "managed care," Erie Family Health Center is busier than ever. Erie's Director of Health Promotion, Ana Cesan, explained, "Erie has expanded in size and scope, which in turn, has doubled the number of referrals for mammograms that we receive. Our goal is to make sure that patients do not fall through the cracks by providing navigation and closing the loop on getting patients access to care."

As a Komen grantee, Erie Family Health Center not only provides patient navigation and manages mammogram referrals, it also runs support groups for breast cancer patients and survivors. In addition, Erie focuses on eliminating barriers to care including: providing transportation, translation services, childcare, food and medication vouchers.

erie family health centerAna noted that Erie Family Health Center holds quarterly events for its constituents to provide wigs, bras, facials, and haircuts. "Our events have been very impactful, and really help to support those in need," Ana highlighted.

Erie Family Health Center provides culturally-competent care to a variety of cultures and communities across the Chicagoland Area, and most of its providers are bilingual in English and Spanish. Learn more»

Did You Know

Recent studies suggest that many women in the U.S. are:

  • Getting their first mammogram later than recommended;
  • Not having mammograms at recommended intervals; or
  • Not receiving appropriate and timely follow-up of positive screening results.

This may lead to more advanced tumor size and stage at diagnosis.

BUT, early detection and effective treatment have resulted in a 34% decline in breast cancer mortality in the U.S. since 1990. In other words, early detection saves lives!
